Honorable mention goes to Guided by Voices' album Propeller. The record was pressed in limited numbers, and each member of the band and their manager hand-crafted the sleeve. Each record made in the original pressing is therefore completely unique.

the anarcho-punk movement had some pretty awesome cover art.
these 2 are from death-punk band Rudimentary Peni. the singer/guitarist (Nick Blinko) did all the artwork in all the albums. this guy (still?) suffers from schizophernia (spelling?) and was hospitalized for it at one point. either way, they have some of the sickest artwork ever. also check out Crass' artwork. pretty heavy stuff on those.
