i think that the intro is like this:

I'm pretty sure that you're off on the intro. It wouldn't make sense to move all the way down to the second fret.
Also, the chords you put are totally atypical of the Sawao's playing style, and it may just be the low audio quality, but right now I'm only hearing two chords in the verse.
edit: maybe these? this is what i'm getting from the ending, anyway.

For the prechorus, you'd just do plain three note power chords, that added fourth note isn't there.

it might be that he's not playing any chords and he's just playing the bass notes with a lot of fuzz/overdrive, because i'm pretty sure i have the rhythm right.

Also, I came up with my suggestion for the rhythm guitar based off the outro, and like I said, the chords you put are definitely not ones Sawao would use. He might even just be doing intervals for the verses, something like --76-- --54--

He's definitely not just hitting a single note there, but it's more stylistic of Sawao to just do standard three-note power chords.

yeah, that sounds better.
and i think you have the verse right too; although i still think i'm hearing another change. after the final chorus, the outro, Sawao strums the chords differently, and it sounds like there are three changes. possibly: --76-- --77-- --54--
and during the pre-chorus, i think there's another guitar that's set on a clean tone... so maybe that's Manabe playing the distorted chords, and Sawao is just playing the chords open?

I think I can sort of hear what you're talking about in the prechorus, but not well enough to make out if he's playing the same chords or different ones. I still can't hear that middle chord in the verses, though it makes sense as a progression. It's hard to hear, but I think it'd be the same as the outro part, which sounds like just two chords.

From the high quality PV file, it sounds like Sawao does just one chord muted over and over again during hte prechorus.
